There could be so many answers to this, but I will pick a few that came to mind.
One Piece and far too many of its contemporary Japanese fictions struggle greatly with the 'culture of perversion' that suffuses the core audiences of anime and manga.
House M.D. is tragic to me with how close to greatness it gets, stymied by its refusal to let the main character enjoy any permanent character development in a positive direction, despite many attempts.
The Rookie has been plagued by this as a vicious cycle, with its key black actors demanding (quite reasonably) that it more clearly address the systematic racism built into the profession it portrays, it attempting to do so, backtracking when its core audience of police-fans reacts poorly to the profession being portrayed at all negatively, thus pushing away those black actors and having to awkwardly cut the characters in the pilot of the next season when they quit.
